Dermatology Associates is a private dermatology practice that participates with most major insurance carriers and Medicare. Due to the increasing number of insurance plans, our office is unable to guarantee coverage by any individual plan. Your insurance policy is a contract between you and your insurance carrier. It is your responsibility to verify that Dermatology Associates of Seattle is a member of your PPO or HMO network before coming to our office for treatment. You can verify this information directly by calling the customer service number located on your insurance card. Also, please verify if your plan requires a REFERRAL from your Primary Care Physician before visiting a Dermatologist. A REFERRAL and/or prior authorization is required and needs to be in place prior to scheduling your visit or treatment.

It is our expectation that you provide us with a correct address, phone number, photo ID and a copy of your current insurance card at each visit. This will enable us to accurately bill charges on your behalf and protect you against identity theft.

If you are a new or established patient without insurance coverage or without proof of insurance payment is expected at time of service. Our office accepts all major credit cards, cash, check and debit card transactions.

  • Billing

    Our billing is conducted by a third party company called ModMed RCM. For any questions or concerns about charges on your billing statement please begin by calling ModMed RCM at 1-866-410-2026. Our office is able to receive payments over the phone or you can pay online by clicking the icon below.

  • Insurance

Also, please verify if your plan requires a referral from your Primary Care Physician before visiting a dermatologist.     We are currently closed to NEW patients for the following insurance carriers:
    Molina, DSHS/Medicaid, CHPW Community Health Plan of WA, Community Health plan – Apple Health.
  • Insurance Referrals

    Benefits may vary, it is your responsibility to know the rules and regulations of your insurance coverage. If a REFERRAL is required to see a Dermatologist, it must be obtained prior to your visit from your primary care physician. Failure to obtain a referral will leave you at risk for payment of all charges associated with any appointment you schedule that is not approved by your insurance carrier. Your insurance company can be contacted by calling the toll free customer service number on the back of your card.
  • Lab+ Pathology Billing

    You will receive prompt notification of any results that are abnormal or require immediate attention, otherwise we will notify you as soon as possible. If you have any concerns after tests or biopsies have been done and we have not contacted you, please contact our office immediately. Lab tests collected and performed by an outside laboratory will be billed separately and are the patient's responsibility. Pathology tests performed by Dermatopathology Northwest or Cellnetix are also billed separately and are the patient's responsibility. Insurance plans have many benefit levels and only you can be sure that your insurance company processed your claim according to your plan provisions.
  • Payment Options

    Payment Options While we try to work with most major insurances there are services that aren't covered by insurance companies and may require self pay. Patients are financially responsible for deductibles, co-payments, and remaining balances. For your convenience our office excepts all major credit cards, cash, check and Care Credit.
